Effect of Injury and Reconstruction on Physiologic and Kinematic Function of the Knee Both laboratory and in vivo investigations have shown that current techniques for reconstruction of the ACL do not duplicate the normal mechanics of the anterior cruciate reconstruction of the ACL do not duplicate the normal mechanics of the anterior cruciate ligament. 22-29 While it is thought by many that the main role of the anterior cruciate ligament is to provide anterior stability to the knee, this is actually a secondary consequence of its spatial orientation in the knee. In fact, in combination with the posterior cruciate ligament, the main role of the anterior cruciate ligament is to provide kinematic guidance. The principal kinematic characteristic of the normal anterior cruciate ligament is a sequential recruitment of the structure’s fibers throughout the entire flexion/extension cycle, which is defined as a pattern of isometricity. Current reconstructive techniques, regardless of the source of the graft, ,replicate this pattern of isometricity. /// http://www.breg.com/research/abstract/FunctionalKnee.pdf (anglais) |
